Please be aware that these new insurance benefits CANNOT be combined with our current in-house Friends and Family Discount Plan.

What’s included?

One exam (which can include an exam from a specialist) and one cleaning is payable once per calendar year.

– Scaling in the presence of generalized moderate or severe gingival inflammation is a covered service and counts as the one cleaning.

– Periodontal maintenance is a covered service and counts as the one payable cleaning.

One set of x-rays (Full mouth serires, bitewing x-rays, or panoramic) is payable once per calendar year.

 

Plan Details:

Maximum Payment: None.

Deductible: None.

Eligible People: All members enrolled in one of the following Security Health Plan medical plans: Spirit, Spirit Rx, Essence, Essence Rx, Esteem Rx, Assurance Rx, Promise Rx, Surety Rx.

What’s included?

Fluoride treatments are payable once per calendar year with no age limit.

Implants and implant related services are payable once per tooth in any five-year period.

All other limitations are listed in section VII of the Dental Dental’s Optional Supplemental Dental Plan Certificate.

 

Plan Details:

Maximum Payment: $1,000 per person total per Calendar Year on all services covered by Dental Dental’s Optional Supplemental Dental Plan. Covered Services included with your Security Health medical plan do not apply to this maximum.

Deductible: $100 per person total per Calendar Year on all services except diagnostic preventive, and basic services.

Eligible People: All members that elect the Delta Dental Optional Supplemental Dental Plan and are enrolled in one of the following Security Health Plan Medicare Advantage plans: Spirit, Spirit Rx, Essence, Essence Rx, Esteem Rx, Assurance Rx, Promise Rx, Surety Rx.
